We welcome new patients living within our practice area
(please see the map of our practice area if you are unsure).
You will be asked to complete a GMS1 form. Please bring photo ID such as a passport or photo-driving licence. You will also be asked to complete a New patient Questionnaire as it may be some time before your medical records reach us. Please print this off, complete it and bring along to the surgery when you register.
There is a hearing aid loop available – please ask at reception.
The Surgery premises and all consulting rooms in our practice are easily accessible for patients with disabilities. Most consultations take place on the ground floor and a lift is available for any consultations carried out upstairs.
